Facility teams spend a great deal of time and focus on falls. The organized involvement of the front-line staff, care-plan team, and family is critical to sustaining success in identifying risk and implementing the right interventions to prevent falls. Join us as we explore realistic teaming, communication, and monitoring systems for keeping those we serve safe from falls.

Objectives:
By the end of this session each participant should be able to:
  1. Implement daily communication and trans-disciplinary teaming systems for fall prevention.
  2. Conduct daily stand-up meetings to help the facility team achieve targeted goals for fall reduction.
  3. Implement daily walking rounds for falls.
  4. Implement a falls focus exercise group for cognitively impaired residents who are frequently falling.
  5. Improve and monitor compliance with care plan interventions to prevent falls.

This presentation includes lecture, humor, group discussion, and individual action planning. The targeted audience includes CEO’s, Directors of Nursing, Rehabilitation and Restorative Directors, Quality Improvement Managers, Consultants to Long Term Care, and all trans-disciplinary care team members.
